Successfully Saved A Women Attempted Suicide by Allegedly Hanging

A 29-year-old woman attempted suicide by allegedly hanging herself on 03.05.2022 around 7 pm at her home. She was found in an unconscious state and did not respond to any stimuli. Her husband rushed her to a nearby hospital. There they continued CPR, intubated her and administered Inotropes with mechanical ventilation. She was referred to Medicover Kakinada in an unresponsive and unconscious state.

Dr.L.V. Ramakrishna Akkina said,“A cervical collar was placed to her neck, and an Endotracheal tube was found In-situ. Hanging marks were observed on the front side of her neck. Her respiratory rate was 30/minute, SPo2- 90% on Bain’s circuit, HR- 140/Mt, BP- 90/60 mmHg, GRBS- 290mg/dl and GCS- E1VetM2. CT scan and other blood investigations were done. On Auscultation, bilateral crepitations were present in both lungs. She got diagnosed with neurogenic shock, neurogenic pulmonary oedema, and hypoxic-ischaemic injury. She had a volume control mode ventilator connection. Drugs such as Levipil, Methylprednisolone, Piptaz, Pantop, Dalacin, Mucomix nebulization, Glycopyrrolate, Midazolam, Amikacin, Metrogyl, Lasix, clexane, and Noradrenaline infusion were used to treat the patient.The vitals improved, while noradrenaline levels dropped. Reports from the CT-C spine showed no fractures. However, compared to lower limbs, upper limb motor impairment remained and was more severe. On the T-piece trial, easing off the mechanical ventilation was attempted, but it failed. Ventilation was kept up in order to prevent hypoxemia, and a tracheostomy was performed due to the extended ventilation.”

After one week, the patient was conscious and responded, moving all four limbs with good physiotherapy support. On the eighth day, mechanical ventilation was stopped, and the patient was on an oxygen mask. After two days, she was shifted to the room.

A week later, the patient was discharged. After three months of follow-up appointments, she fully recovered. The family thanked the Medicover ICU team and the physiotherapists for the excellent efforts in recovering her faster.
