Introduction: Scar ectopic pregnancy is becoming increasingly common all over the globe. It is novel and life-threatening form of abnormal implantation of embryo within the myometrium and the fibrous tissue of the previous scar following cesarean section; hysterectomy; dilation and curettage; abnormal placentation; surgery on uterus like myomectomy, metroplasty, hysteroscopy and manual removal of placenta.
Case Report:
This month we would like to share a case of a 30yrs female, who came to ER with active bleeding P/V. After performing dilation & curettage, after spontaneous abortion with retained placenta in a peripheral hospital, referred to medicover hospital, patient in G3 P2 L2 with 2 previous LSCS.
Our diagnosis was SCAR ECTOPIC and her vitals were stable, reserving 3 PRBC’s, 3 FFP’s. Ultrasound revealed scar ectopic. We proceeded for surgical management. Our plan was scar resection with uterine reconstruction.
Under general anesthesia we proceeded for surgery. On the table, diagnosis was confirmed it was an exophytic type of scar, ectopic bladder separation was done, bilateral uterine arteries ligation done and adherent placental tissue was removed.
Hemostasis secured, proceeded with uterine reconstruction, abdomen closed in layers with drain left in-situ, throughout procedure vitals were stable. Patient shifted to ICU for 48 hours; there was no active bleeding P/V, patient vitals were stable, recovery was uneventful and was discharged on POD-5.
